Introduction: the presence of the nuchal cord is frequent during delivery and does not usually cause adverse fetal or neonatal outcomes; but in case of multiple loops, the risk of perinatal complications or death is increased if timely management is not done.
Case presentation: we report a case of a 30-year-old G3P2 with a past history of nuchal cord in her previous pregnancies, one resulting in neonatal death and a previous cesarean section. She delivered at 40 weeks of gestation through an elective cesarean section of a female child with four loops of nuchal cord and severe neonatal asphyxia. The newborn baby was admitted to the neonatal and resuscitated. We review the recommendations about the management of multiple loops nuchal cord and highlight the lesson learned.
