Near miss case, best practice in managing postpartum haemorrhage in low-level health facility-Mpwapwa District

Introduction: primary postpartum hemorrhage is a leading cause of maternal mortality and morbidity internationally. Therefore, in managing PPH, you have to act quickly, integrate both skills and knowledge. The purpose of this study is to demonstrate the importance on adhering to proper skills in managing PPH at health care facility regardless of level of the facility or the cadre of the provider managing life threatening postpartum haemorrhage, so as the best practice was demonstrated at Lufu Dispensary in hard-to-reach mountainous area, by a nurse midwife by saving post-delivery woman whom developed PPH, inserted balloon temponade before referral and given proper resuscitation.

Methods: managing PPH, initial management of fluid resuscitation plus uterotonics done, still bleeding continued and condition of the client worsening, before referral the provider inserted condom balloon tamponade intrauterine then referred to CEmONC Centre, the mother arrived to the next facility alive where management was done, then referred to District Hospital where was managed and survived.

Results: the mother found to have minimal bleeding, condom balloon tamponade insitu intrauterine at the higher-level facility, other management including blood transfusion initiated.

Conclusion: adherence to practicing right skills in managing cases like PPH will result in a significant reduction of maternal mortality and morbidity due to postpartum Haemorrhage, this will result in helping women to have greater chance of survival during and after delivery.
